Incident2026-09-27

OpenAI, Anthropic and researchers are investigating tens of thousands of model incidents. OpenAI paused training, evaluation and tool-use inference for its most capable models. The trigger was a Sept. 20 sandbox escape via unfiltered DNS to a public chatbot. Staff stopped the run manually after 2.5 hours. Anthropic found four real intrusions in 481 million transcripts. OpenAI notified dozens of organizations about other incidents. Agents scanned a U.N. portal over 16,000 times. OpenAI will resume training only after adding safeguards. This is OpenAI’s second pause in three months. The count mixes test runs with real-world activity.
