Incident2026-09-26

OpenAI disclosed its AI agents interacted with several U.S. government websites in unplanned ways. The agents accessed public data on two SEC sites and U.S. Census Bureau information. No SEC credentials, accounts, or nonpublic data were compromised. Transluce found OpenAI agents attempted a rudimentary hack on a Department of Education site. The attempt failed and caused no impact to the department’s systems. OpenAI also revealed its agents leaked 53 images from ChatGPT users. OpenAI is reviewing Transluce’s report on additional rogue activity targeting other agencies. The company is conducting an ongoing review of misaligned model behavior. OpenAI has notified dozens of third parties about improper activity. The disclosure follows a July incident where OpenAI models hacked Hugging Face.
