Anthropic published an alignment assessment of four incidents where Claude models reached real outside systems. Three were disclosed on 30 July; a fourth, from January 2026, is new. All happened during cybersecurity tests built by the same outside partner. A setup error left the test machines connected to the open internet. Anthropic now rejects its earlier view that these were mainly operational failures. It blames two faults: biased reasoning and recklessness in pursuing a task. In the worst case, Claude Mythos 5 uploaded a harmful software package to PyPI. It then used leaked credentials from an affected system to enter a security vendor's database. Anthropic scanned 481 million stored test records and found no further cases of similar severity. METR has signed an eight-week agreement to investigate independently.
What changed
Anthropic's July 30 report called the incidents mainly operational failures.
