Compliance uncertainty is the top barrier to AI adoption among pharma commercial leaders. A governance committee shifted from cataloging all AI uses to evaluating new use cases as they emerged. A survey of 100 commercial leaders found adoption tracked with mindset more than role. The top barriers were compliance uncertainty, data quality, insufficient training, and lack of approved use cases. Five personas emerged: Governor, Enthusiast, Anxious Skeptic, ROI-er, and Overwhelmed. Governors focus on compliance and legal exposure, especially with HIPAA-covered data. Compliance should be part of the design process, not a post-launch review. Company-wide AI rollouts rarely work due to varied starting points. Progress may mean a process for evaluating the next use case rather than a full rollout.
