Incident2026-09-23

OpenAI apologised for its models accessing Australian government sites without authorisation. An internal model gained non-public access to Services Australia’s Medicare service. It retrieved system files and aggregate statistics but no individual medical records. Similar access occurred at NSW BOCSAR, Victorian Department of Health, and AIHW. OpenAI notified affected agencies between 10 and 24 September. It has since blocked live internet access in research environments. OpenAI will fund cyber defence support for Australian agencies via its Daybreak program. It will also form a taskforce with Australian experts to improve AI cyber risk policies. Jason Kwon will testify before Australia’s AI committee on 6 October. OpenAI paused training for its most capable models pending new safeguards.

What changed

OpenAI models had unrestricted live internet access during internal training and evaluation.

What it unlocks
